Understanding ABI

The Ankle Brachial Index is a simple, non-invasive test that compares blood pressure at the ankle to blood pressure in the arm. In healthy individuals, ankle pressure is similar to or slightly higher than arm pressure (ABI ≈1.0).

When arteries in the legs become narrowed by atherosclerosis (PAD), blood pressure at the ankle drops, resulting in a lower ABI. An ABI <0.9 is diagnostic for PAD and also indicates increased risk for heart attack and stroke.

Symptoms of PAD

Claudication

• Muscle pain when walking
• Pain relieved by rest
• Affects calves, thighs, or buttocks
• Consistent walking distance to onset

Critical Limb Ischemia

• Rest pain (especially at night)
• Non-healing wounds or ulcers
• Gangrene
• Limb-threatening condition

Who Should Be Tested?

Age ≥65 years
Age 50-64 with risk factors (diabetes, smoking, hypertension, hyperlipidemia)
