The calculator shows your possible final grade range based on remaining assignments. The minimum assumes 0% on everything left, while the maximum assumes 100%. Your actual grade will fall somewhere in between based on your performance.

This shows how much of your final grade has been determined. If it says 'Based on 60% of total grade,' you've completed assignments worth 60% of your grade, and 40% remains. Your current grade only reflects the completed 60%.
Yes, if you have high-weight assignments remaining. If you've completed 80% of your grade, remaining assignments can only change your grade by up to 20 percentage points. If you've only completed 40%, much more change is possible.
