Both work well. OSB is cheaper but swells more if wet. Plywood is more moisture-resistant. For wet areas (bathrooms, kitchens), use plywood or AdvanTech.
Highly recommended. Adhesive prevents squeaks and adds structural strength. Use a subfloor adhesive like Liquid Nails LN-902 or PL Premium - one tube covers 4-5 sheets.
Yes, but screws are better. They hold tighter, are less likely to back out, and make it easier to replace damaged panels. Use ring-shank nails if you must nail.
Subfloor is structural (3/4" thick, fastened to joists). Underlayment is thinner (1/4"-1/2"), goes over subfloor to provide a smooth surface for flooring, especially tile or vinyl.
From below: drive screws up through subfloor into finished floor. From above: drill pilot holes and drive screws through finished floor into joists. Use breakaway screws for carpet areas.
