Reticulocytes are immature red blood cells recently released from bone marrow. They retain residual RNA that can be stained, allowing them to be counted. The reticulocyte count reflects bone marrow erythropoietic activity.
In anemia, the reticulocyte percentage may appear falsely elevated simply due to fewer mature RBCs. The corrected reticulocyte count and RPI adjust for this and for the fact that reticulocytes are released earlier and take longer to mature when anemia is severe.
